Presbyterian SeniorCare Network | 2022 COMMUNITY REPORT 7 Access to healthcare is fragmented, complicated and overwhelming; this is especially true for older adults. Last year, Presbyterian SeniorCare Network committed to taking on this problem, and, with a funding grant, established the Center for Innovation and Care Transformation (CICT). The mission of CICT is to ensure older adults get the care and services they need, when needed, in the way they choose , both within Presbyterian SeniorCare Network and beyond. CICT is actively collaborating, creating partnerships and innovating to demonstrate improved outcomes through a partnering approach with older adults, their families and caregivers. Evaluated, Explored and Launched Initiatives In little more than a year, CICT was launched and has evaluated more than 30 opportunities, including the selection of 12 for further exploration and the following six initiatives for the first wave of development and implementation: 1. Advancement of the Dementia360 Care Management Program to support caregivers of and individuals living with dementia 2. Commercialization of the Dementia Care Center of Excellence to improve dementia care more broadly 3. Internal Adoption/Alignment of Care Management Framework 4. Affordable Housing Care Management Program 5. Engagement in insurer value-based payment programs and alternative payment models 6. Skilled Nursing Facility/Home Health Pilot reducing gaps in care and fragmentation CICT aspires to help position Presbyterian SeniorCare Network to be the market leader that provides unique care management models to manage the chronic conditions of the age 65+ population. CICT also endeavors to scale the Network’s infrastructure and targeted programs to address the projected dramatic growth of the 65+ Baby Boom generation, with a particular focus on individuals 85+ years of age. The Network’s 2030 strategic work plan is built upon this vision.
