6 I n 2022, Presbyterian SeniorCare Network received a $500,000 HEALTHY AGING CHALLENGE GRANT FROM THE HENRY L. HILLMAN FOUNDATION . The grant is supporting the Collaborative Healthy Living Model, an initiative to help older adults to receive primary care in the Network’s affordable housing communities. “We understand the healthcare system is overwhelming, complicated and frustrating—especially for older adults,” said Celeste Golonski, Senior Vice President of Strategic Initiatives for Presbyterian SeniorCare Network. “Integrating service coordination with a primary care clinic inside our apartment communities removes significant barriers and helps older adults navigate the healthcare system more easily. Our goal is for every senior to get the care and services they need, when needed, in the way they choose.” The Network’s new service emphasizes collaboration among service coordinators, primary care providers and residents to prevent hospitalizations and create better outcomes. Early intervention and self- management will be emphasized, preventing the need for a higher level of care and supporting residents to age in place with grace and dignity. The first healthcare clinic opened on February 27, 2023 at Bellmead Apartments, an affordable housing community in Washington, with more clinics expected to open throughout the year in affordable housing communities across the Network. The new, onsite clinic means Bellmead residents can make doctor’s appointments and visit the clinic without having to leave the building. Residents of two nearby communities operated by the Network—HaveLoch Commons and Heritage House of Houston—can schedule appointments to receive care directly inside their apartments. Curana Health, a primary care medical provider group, is partnering with Presbyterian SeniorCare Network in this endeavor. For the Healthy Aging Challenge grant, the Network and three other finalists were selected through a review process that included peer-to-peer evaluation among the 29 applicants and review by a panel of 27 local and national experts and industry leaders.
