The LPN Case Manager responsible for conducting thorough assessments of residents’ medical and care needs with the overall goal of enhancing the quality of resident management and satisfaction.  The position promotes continuity of care and cost effectiveness through the integration and functions of case management, utilization review and management, and discharge planning. As a key support member of the skilled nursing leadership team, candidates are required to demonstrate excellent customer service behaviors through effective communication with team members, residents, and family members.

What we are looking for:

We need a proven clinician who will not only help our team members and Network leaders as they support the daily living needs of our residents, but who also desires to develop trusting, caring relationships. The LPN Case Manager is required to:

  • Be a graduate of an approved School of Practical Nursing and have a minimum of 1 year of case management experience;
  • Possess a valid LPN license issued by the State of Pennsylvania and maintain current CPR certification, per policy;
  • Demonstrate strong interpersonal skills to support, advise, coach, and influence others while modeling enthusiasm and motivation;
  • Be self-directed, set priorities, and have excellent time management skills;
  • Exhibit proficiency with EMR software as well as PC and Microsoft Office applications. 

Prior experience in a skilled nursing community is preferred.
