The RN Assessment Coordinator is responsible for directing and coordinating the residents’ medical and care needs, providing educational training for nursing staff with regard to the minimum data set, ADL documentation and case mix, and ensuring accurate documentation for reimbursement purposes. This position plays a lead role in coordinating care delivery, monitoring resident progress, and maintaining compliance with regulatory requirements. As a key support member of the skilled nursing leadership team, candidates will be required to exhibit excellent customer service behaviors and interactions through effective communication with team members, residents, and family members.

What we are looking for:

We need a proven leader who will not only help our team members and Network leaders as they support the daily living needs of our residents, but who also desires to develop trusting, caring relationships with the residents they support. The RNAC is required to:

  • Be a graduate of an approved Registered Nurse School of Nursing and have a minimum of 1 year of RNAC experience in a long-term care setting; 
  • Possess a valid Nursing license issued by the State of Pennsylvania and maintain current CPR certification, per policy;
  • Have at least 2 years of experience with completing MDS and facilitating interdisciplinary care planning;
  • Possess strong interpersonal skills to support, advise, coach, and influence others;
  • Exhibit proficiency with MDS software as well as PC and Microsoft Office applications. 

Prior experience in a skilled nursing community is preferred.
